What you’ll need:

• Completed physical form DOH-3122 (Must be completed by a physician)
• Complete mental health evaluation form DOH-5075, if you have a mental health diagnosis (including anxiety / depression – Must be completed by a physician)
• Negative PPD Test or Chest X-Ray dated within 30 days of your application date
• Last 6 months of bank statements

It’s helpful to have:

• Award letters for income sources (Social Security, Veterans’ Benefits, Pensions, Workers’ Comp, etc.)
• Proof of previous vaccinations
• Photo ID & Social Security Card / Birth Cert.
• Copies of any insurance cards (Medicare, Medicaid, Etc.)
• Copies of Advance Directives (Health Care Proxy, Power of Attorney, MOLST)
• Copies of Life Insurance Policy (if applicable)
• Copies of Marriage / Death / Divorce Certificates (if applicable)
• Vehicle Registration & Insurance (if applicable)
• Verification documents for any cash / investment assets
